(BATCI) para sa mga nais mag enroll sa kursong Animal Production (Ruminants) NCII na tatagal ng 39 days: 1 Filipino, male or female, 21-59 yrs old 2 Able to read & write 3 Physically & mentally fit 4 High school report card-Form 137/HS Diploma/transcript of Records (college) 5 PSA Birth certificate 6 Brgy. Clearance 7 Photocopy of passport 8 Pictures: 5pcs passport size (formal attire, white background), 2pcs 2x2, 3pcs 1x1 9 Medical: You sent a week ago 9 Medical: a. Rapid Test (pls ensure that YOU STRICTLY follow health protocols during & after the test) b.TB & Hepa B tests c. CBC, Fecalysis, Urinalysis d. Chest x-ray e. Medical certificate showing medical history & current medication if any f. Accomplished Health Checklist (to be provided by BATCI) Training cost: 35,200 (if your not aTesda Scholar) - tuition, health & wellness, board & lodging (with 3 meals/day), uniform, misc fees. You sent a week ago Enrollment process: 1. Make sure Requirements 1, 2, 3 are fully met 2. Secure medical tests; pls prioritize Rapid Test. a) We have no accredited testing center. b) Pls ensure that address or phone numbers of testing center is reachable so we can verify the test results. 3. Once you receive Rapid Test result, pls email a copy to bulacanagricultural@gmail.com 4. A Health Checklist will then be immediately sent to you. Pls accomplish the checklist and email it back to bulacanagricultural@gmail.com 5. Once we receive the accomplished Checklist, we will send instructions to complete your enrollment. Sa mga nagnanais na mag-avail ng TESDA scholarship, maari lamang na iaccomplish muna ang mga nabanggit na requirements.
